Applying a passport for a minor requires you to have Annexure, but depending on each situation, the Annexures are different. What is Annexure C for minors’ passports?

A passport is an essential travel document for those who are travelling abroad for education, pilgrimage, tourism, business purposes, medical attendance and family visits. During the past few years, the growing economy and globalization have led to an increased demand for passport and related services. To meet this demand, Ministry of External Affairs has simplified its procedures for issuing passport for minor children, according to Regional Passport Officer S. Maniswara Raja. The change in procedures has come into force from September 1.

How many types of annexures for an Indian passport?

Till now, there are 14 types of annexures that Indian passport holders can apply:

• Annexure A: Affidavit of date of birth to be submitted by illiterate applicants.
• Annexure B: Identity certificate.
• Annexure C: Affidavit for a passport of a minor by either parent (who are separated but not formally divorced).
• Annexure D for passport for Indian citizens: Affidavit for lady applicant for change of name after marriage.
• Annexure E: Change in name/deed poll/sworn affidavit.
• Annexure F: Verification certificate for tatkal passports.
• Annexure G: Declaration of parent/guardian for minor passports (consent from one parent).
• Annexure H: Declaration of parent/guardian for a minor’s passport.
• Annexure I: Standard affidavit.
• Annexure J: Specimen verification certificate.
• Annexure K: Specimen affidavit for issue or re-issue of passport that was obtained prior to marriage, by married applicants who are unable to provide the prescribed marriage certificate or joint affidavit with spouse due to marital discord, separation or without formal divorce decree by the court or due to total desertion by the spouse.
• Annexure L: Specimen affidavit for a passport in lieu of lost/damaged passport.
• Annexure M: No objection certificate.
• Annexure N: Prior intimation letter.

What is the meaning of Annexure C?

Annexure C specimen declaration by applicant’s parent or guardian for issue of passport to minor when one parent has not given consent. Annexure C is applicable where the details and whereabouts of the other parents are not known. The single parent with whom the child is residing may submit Annexure C. Besides the local guardian of the minor child, if he or she is not the grandparent of the child, one has to submit an affidavit with two responsible persons.

Who Should Make Annexure C?

When the following situation satisfied, then Annexure C can be enclosed.

• If you are a single parent, either the father or the mother of the minor child and wishes to apply for minor passport for your child
• If the minor’s other parent does not give consent to apply or sign in for the passport

What Are The Details Required To Create Annexure C?

• State whether you are the father or mother of the passport applicant
• Mention the name, address and other details of the minor applicant
• Mention the other parent’s name who is unwilling to provide consent for the passport application.

Documents Required for Applying for Minor Passport as Single Parent in India  

The documents checklist is same, if you are applying as a single mother or father. It also is same, if you are applying as guardian as well.

• Proof of Date of Birth (DOB): Birth Certificate issued by Municipal Corporation or similar government office that can issue birth certificate. If you do not have birth certificate, apply for birth certificate and get it ready to avoid issues. It will really make your life easy, if you have all the spellings correct in the birth certificate.
  • Adopting Child: If let’s say you are adopting the child and applying as single parent, and the birth is not properly documented, then You need a letter in the form of “A declaration given by the Head of the Orphanage/Child Care Home on their official letter head of the organization confirming the Date of Birth of the Child”
• Proof of Present Address:  You need current address proof of where the child would live along with the parent/ guardian, who is applying for the passport. Some of the acceptable documents are like Water Bill, Electricity Bill, Proof of Gas connection, Aadhar Card, Rental Agreement, Parent’s passport copy with the same address,  etc.
• Declaration – Annexure C: You need to print out Annexure C from Passport office website on plain white paper and fill out the details and sign the same to submit when you visit the Passport Seva Kendra. There is no need to go to any officer or notary to get this signed. You just need to sign it yourself as the parent or guardian.
• Baby Photo: If you are applying for young child or baby below 4 years of age, you need to carry a passport photo ( 4.5 X 3.5 cm) of your baby with white background. It will be mentioned on your appointment letter.
• Copy of Parent’s/ Guardian’s Passport: if you have passport, you can carry a copy of your passport and sign it as well. This is not a mandatory document, but sometimes they may ask.
